If you can tell us more info about the power supply you used we can make a better idea of the problem, cause 12V should not damage in that way the module. We think that probably you used an inverted power supply.
I would suggest you to try to put it back in the eurorack to see if it still works in there.

So, we've got two discoveries.
First, I've checked both adapters possibly plugged in to FX KIT. One is ~12,3V, the other one ~15,5V, even if the label says 12V (ugh, great...). One of those two was plugged in.
Second, I've found this on the lower board (images below).
No working: looper/shifter, CV interface, CV generator ( + no lights at all)
Mixer, delay and reverb works fine. Of course, it's impossible to control CV input routing to selected parameters (selecting diode color).
FX was bought used, but worked fine. I don't know if this scratch on the lower board was caused be me.
So, we have 3 possible reasons:
1. 12V "+" in center
2. 15V "+" in center
3. Physical board damage
An analogue 'half' of the FX KIT still works (wow!).
